I hope that it will help you finding a quest. I also notice that when you go to help other known survivors hunting zeds or looking for a friend survivor, in middle of your way, Lily will Radio message you a quest (either main or side quest).

Just want to pin point what the problem might be. Okay let me see if I understand it well, the 75% of the city in the South you mean the first base where the Church is? the 90% of the small on in the North is where you start the game at the Ranger's Base? and the city that you don't have access in the way West is the biggest city that you see on the map or is the one that in order to enter you have to pass from a bridge with a gate (the army area)?
